1. Please fill out the medical application form and submit it with the insurance card (various medical certificates) and medical referral letter. 2. We will issue a patient card and patient ID card (We will call you by name to prevent misidentification). 3. Please confirm your name, birth date, address, telephone number at the reception. 4. Please bring your referral from a clinic or doctor’s office.
